Appeal for change of school

My daughter has been assigned to SM Subang Utama which is not our choice. It's inconvenience for us since we are staying in USJ6. Can anyone share with me where do we go to make an appeal for change of school? What are the documents required to for four the appeal?

You need to go to the assigned school and GET THE APPEAL FORM, can go and do it today, SMSU Office is open today (9/11/2015). All is written in the form.
