Dear folks,
I am now in the midst of revamping my clinic services and will be launching some new products & laser machines on July 27th next month. You will be receiving an invitation for that shortly.
I will also be sending out weekly (or bimonthly) snippets of information on the Science Against the Effects of Aging. I aim to provide informed, unbiased information in the field of anti-aging research & new cosmetic innovations. The topics selected will frequently be based on the questions asked by my patients. Some of the mail-outs will also be informing you all about any new products or services I have in my clinic (there has to be some sales talk!)

What is an appropriate longevity diet? What should I eat for my skin condition?
Below I have listed a "must buy" shopping list which I have compiled based on foods which have been well-researched and shown to have beneficial effects on anti-aging, anti-cancer, anti-oxidant etc. The list is by no means exhaustive, but it's a start!
Fruits & veges section (just about everything in this section of the supermarket have some medicinal gems)
- garlic, onion, scallion, leeks, chives
- cruciferous greens - brocolli, cauliflower, cabbage, brussel sprouts
- ****ake mushrooms
- green leafy vegs e.g. spinach
- orange veg - carrots, sweet potatoes, pumpkin, papaya, mango, cantaloupe
- red vegs - tomatoes
Meats
- Fish esp salmon ( sharks, tuna reputedly have too much mercury on board)
- Lean chicken or turkey
- lean red meat
no viscera!
Dairy
- low fat yoghurt (plain! mix it with fresh fruits - more palatable)
- Yoghurt drinks. Vitagen etc has far too much sugar, but if it is the only source of probiotics that the kids will consume, it's better than no consumption at all!
- Low fat milk
- No butter or margarine
Drinks
- soyabean /taufoo
- green tea (oolong tea for eczema sufferers)
Cooking oil - olive oil
Bread - whole grain
Rice - brown unpolished rice
Eggs - contrary to popular belief is good for you - excellent source of lean protein.
For those of you who shop in Chinese herb shops, ginseng, gingko & dong quai now have very solid scientific evidence of health benefits. Chinese herbs are another source of great medicines, but it's just that not enough Western-conducted studies have been done to give it the recognition that it deserves. When buying Chinese medicine, try to buy the real roots & plants to avoid steroid contamination in tablets or capsules.
My question is ... how do you creatively cook all these healthy foods to make it a gastronomic feast? If anyone has any recipes to share, I would appreciate if you could send them to me!

Dear all,
Welcome to my 2nd e-newsletter. Firstly, some house-keeping. One of my patients mentioned that I omitted legumes from last week's shopping list. Indeed, I did. Legumes (beans, etc) are a very important food group which can help alleviate diabetes, high cholesterol; it also has some anti-cancer properties and has plenty of fibre to help constipation. So, add that to the list!
This week's topic deals with placenta "live cell therapy". For those of you who have never heard of it...briefly...."Live cell therapy" is an injection of fresh cells (commonly obtained from sheep placenta, some unreputable brands use human placenta from China!) into your body to stimulate cell renewal. Anecdotal benefits include finer, even-toned skin, firming up of skin & sagging breasts. boosting immune systems, improving sleep, enhancing energy & stamina, improving blood circulation, easing menstrual & menopausal symptoms and boosting sex drive. This has been famously practised in Clinic La Prairie in Montreaux, Switzerland. It has a big clientele consisting of the very rich & famous around the world. To enhance international business, they have also come up with an oral encapsulated version which is also anecdotally, effective.
So, what's the real deal? Is this really the magic bullet?
We have to address 2 questions, firstly about LIVE CELL INJECTIONS.
1) Is it really effective?
First of all, there are no studies in peer-reviewed journals to support claims of it's effectiveness. However, I sincerely believe in it's benefits as listed above. I have heard from unbiased reliable sources (i.e. a Japanese physician friend, not someone locally who is trying to sell me placenta products) who can attest to its effectiveness. Moreover, live cell therapy has been practised for decades and has a very strong following world-wide for those who can afford it. If there are no benefits, people wouldn't pay good money for it.
2) Is it safe?
The short answer is NO. The major complication of live cell injections is severe allergic shock (drop in blood pressure, shutting down of breathing tubes, swelling of the whole face) which can be fatal. Hence, it should definitely not be administered in conditions that are done currrently in Malaysia i.e. at the back of beauty salons by medically-unqualified people. It also being done discreetly at some medical clinics. However, if it is a clinic which has no resuscitative facilities, it is not safe either. Clinic La Prairie in Switzerland is a hospital with full medical facilities, hence can conduct live cell injections relatively safely.
Therefore, I do not advocate live cell injections for various reasons:
Firstly, it is NOT registered by Ministry of Health Malaysia (contrary to a lie that is being propagated at the moment, a brand of live cell/placenta injections called VITA X, is NOT registered with the Ministry of Health (MOH) - this fact I have double checked with the Ministry)
Secondly, the risk of anaphylaxis (=severe allergic shock) is too high and can result in death.
There are only 2 conditions whereby I would advocate live cell therapy - full approval by Ministry of Health Malaysia and administration in a hospital setting. Until then, I am against it.
How about live cell/placenta ORAL CAPSULES?
Well, so far, deemed safe and certain brands have been approved.
There are only testimonials about its effectiveness and since Clinic La Prairie has an oral version, there are probably SOME benefits.
For those who want to try oral placenta capsules, I would choose a brand that has been approved by MOH (approved for safety). Certain brands are derived from human placenta and all sorts of diseases including AIDS can be potentially transmitted.
I would recommend a brand called VC SHEEP PLACENTA EXTRACT - manufactured in Australia, fully approved by MOH Malaysia. It retails for RM368 and is available in my clinic. I do not have any financial interest in this company but I recommend it because it is fully registered and has been tested to be free of heavy metals, steroids and hormones. Dear folks,
My new laser machine has arrived! It is the new generation LYRA i long-pulsed Nd-Yag laser.
It can do the following:
1. Facial & leg veins - Up to 4 mm diameter leg veins can be removed. It can coagulate small vessels in as little as a single session. Larger leg veins will require more (usually 2) sessions. This requires an application of local anaesthetic cream for 1 hour to reduce the discomfort.
2. Type 2 skin rejuvenation i.e. stimulates collagen production to firm up the skin, decrease fine lines, wrinkles, shrink pores, minimise scarring. Compared to IPL (which a lot of you have tried), this laser specialises in type 2 rejuvenation, whereas, IPL does both type 1 (i.e. removal of superficial pigmentation) & type 2 rejuvenation but specialises in neither.
Usually for best results, 6 sessions 3 weeks apart are required. After 6 sessions, skin improvement can last 1 year.
Most importantly, there is no pain (really!), definitely less painful than IPL. But it is slightly more painful on the hip pocket nerve.

3. Permanent hair removal - again not too painful (local anaesthetic cream can be applied for those with a low pain threshold). Advantage of this laser compared to other systems is that it is safe and won't cause burns even for dark skin types or if you have a tan. The laser beam is shot through a cold window, hence the epidermis is protected and the paucity of pain.

Cost is based on laser time i.e. 300 for 5 minutes laser time. I would estimate veins behind knees to take 2 sessions max, total cost anything between 500 -1000, depending on laser time (i.e.no. & length of veins)

Beauty News.
To want to look good is a basic human desire. We are bombarded everyday by advertisements claiming to make us look younger overnight. I'm frequently informed by my clients that beauticians bombard them with a barrage of claims that often induces them to sign up for another RM2,000 course of facials or another thousand on new products. What is true and what is fiction?
Here are some common questions asked by my clients:
1. Do we need to "deep cleanse" our skins?
For most people, water and a mild cleanser used twice daily is sufficient. If you have very dry skin, use a soap-free cleanser. If you have acne-prone skin, you may want to use a cleanser with added anti-bacterial ingredients. If you wear heavy make-up, use a cold cream (makeup remover) before the cleansing. Good cleansers need not cost a bomb; even good brands should cost less than RM100.
Don't believe advertising hype that "deep cleansing" is necessary to rid your skin of dirt and "impurities". Facials are pleasant and relaxing but they don't "restore" or "rejuvenate" your skin. Overzealous extraction is actually harmful and can cause dyspigmentation and scarring.
2. What can I expect from facial masks?
Facial masks can cleanse the skin and leave it looking and feeling smooth for a few minutes to hours. As the mask dries, it produces a tightening, tingling feeling that makes one think something is happening but there is no scientific evidence that skin ageing will be reversed.
3. Do facial exercises prevent wrinkles?
No, they do not; in fact, the opposite is true. Facial movements such as laughing, frowning, etc cause wrinkles. That is why doctors inject BTA ("Botox") into strategic areas to relax facial muscles so that wrinkles cannot form. (BTW, I'm not suggesting that laughing is detrimental to your skin!)
4. Do facial muscle stimulators counteract facial sagging?
I guess it does for a few days but it has to be repeated weekly to maintain the results and is it worth the trade-off of getting more wrinkles? Facial sagging is an inevitability caused by gravity acting on us upright bipedal hominids. I see it only as a cosmetic problem in people who have put on too much weight around the face i.e. it's the fat that is sagging, not so much the facial muscles. People who have maintained a reasonable weight all their lives don't complain much of face sagging. In fact, I have lots of slim 50+ clients who don't complain of face sagging ( but they do so of wrinkles!) The message is, maintain a healthy diet and weight for your skin health AND overall health.
I am frequently told that fat people suffer less from wrinkles. I'm not sure that this is true. I suppose the argument is that fat padding props up the skin to iron out wrinkles. But the fat layer is the hypodermis (the layer below the dermis where the collagen and elastin is). The centre of action for wrinkles is the dermis. Wrinkles and caused by a decrease of collagen & elastin in this layer, so I don't think that fat padding will help in this department.
5. Do moisturizers work?
Well, you need them to lubricate your skin and to prevent dehydration. Moisturizers by themselves do not prevent wrinkles or skin aging unless they contain other active ingredients or sunblock.
6. What is an affordable way of keeping wrinkles at bay?
a) Stay away from prolonged sun exposure. If outdoors, use good sunblocks of SPF 20 or higher. Wear a hat, long sleeves, pants, umbrella... cover up!
b) Start a skin rejuvenating regime of vitamin A & C creams, Kinerase and fruit acids to reverse some of the changes of aging and slow down the aging process. So far, these are the best treatments that have been proven by scientists around the world.
7. What can I do for deeper wrinkles that are refractory to creams?
Further in-office intervention is, of course, more costly, but some people may wish to take that step further in enhancing their appearance.
Procedures available include:
- Peels (chemical or microdermabrasion)
- Fillers e.g. Restylane, Nufill
- BTA injections
- Lyra skin rejuvenation
- Intense Pulsed Light skin rejuvenation
(The last 2 are examples of non-ablative skin rejuvenation)

IPL is not useful for Acne scar . Best is combination of microdermabrasion & Lyra laser.
Micordermabrasion per session is 120, full course is approx 1200 (10-15
sessions).
Laser starts from 200 per session, depends on size of area treated. Need 6-8 sessions.
hmm. how does the lyra laser works.. i haven't heard of that laser before..
oh yah one more question..
i heard that needling are used to treat acne scars.. do they do it in m'sia?

"I have a few scars on one side of my cheek.. i can say its not a serious one..

There are a number of treatment choices for moderate acne scars - needle abrasion, microdermabrasion, filler injections (Nufill, Restylane), laser.
Choice depends on budget, severity of scars, etc. I don't have
smoothbeam...I use Lyra i laser.

Needleabrasion cost is RM2000 (client buys the gadget but procedure is done
in the clinic - can be reused for as many times as desired. Procedure can be
repeated every 3 months)
Approx. 50% improvement is max expected.
Results vary between people, but I think needleabrasion gives better results
versus Lyra laser which is non-ablative.

Subcision is undercutting the scar to free up fibrous tissue. Needle
abrasion is needling the skin (multiple stabs) to stimulate collagen.
Subcision is usually done before introducing a filler under the scar.
Cost depends on surface area covered. Why don't do you just come to my
clinic and show me your face so I can assess it properly?

Some cosmetic products are known to have hydroquinon as its ingredients. There was a commotion about this issue a couple years back.
What is hydroquinon?
Is there any side effect of using these products for a long period of time?
Are they still safe to use or should they be avoided totally even when the cosmetic companies claimed that the hydroquinon content is very mild?

unlike a bad hair do where the worst would be that of the wait for the hair to grow back
if you ask me better you leave your color of your skin as natural looking as it can be
but if you insist it is safer to learn to apply better makeups to suit your emotion or mood when necessary only
you know what happens when the melanine is compromised you might need to put on makeups permanently
i am certainly not the expert in the field but common sense should tell you what you need to decide & do
don't just trust products or its labels
otherwise you might one day loose that pretty sundaygirl face in the mirror

Now let me explain why I persuaded Dr Lim to take part in this Forum indirectly. The purpose was to provide a channel for our lady folks who may need to raise issues of beauty, facial care etc I personally persuaded Dr Lim to help us out because none of us are able to handle such a thread. In return, we do give her a bit of mileage particularly when she can offer something special for our ladies and even the men who care about their looks.
I trust our explanation makes it more palatable now?

patrick - I assume Dr Leng is a medical doctor with an annual practising certificate from the Mmalaysian Medical Council. By listing her services and place of practice on this forum may contravene the professional ethics and should there be any complaint to MMC she can be hauled up and reprimanded. Her services to USJ folks may be noble but this can be interpreted as advertising by a medical practitioner. She should not list her place of practice and used a pseudonym in all correspondence to answer questions instead. You may inadvertently put her in a difficult situation. As a medical practitioner she should know that such practice is prohibited. I am sure you have noticed that when doctors go on radio live, they are not to divulge their place of practice on the air for this reason.

Hi good Drs., MDs.,
Please pardon our ignorance, but the truth is we need an expert medical advice, n we would be forever obliged if you could offer an expert medical advice to us. My wife suffers with a bad case of asthma since her first pregnancy more than 8 years ago. Her asthmatic conditions is getting from bad to worst.
Time n time again, we worry what we post here, could be off topic, but since expert 'beauty' advice is freely available here, we thought we could get a good recommendation from you in our effort to find help for my wife's worsening asthmatic conditions.
Prior to this, we tried everything we possibly could, including sending her to a local large private hospital on a regular basis to seek treatment, but til such time we realized that we could only get treatment to control her asthma conditions rather than finding a good cure for it. That was about 5 years ago. We stopped because the non-effective treatments from the hospital's specialist at the time, was costing us a bigger hole in our pockets.
We even tried traditional chinese herbal medicines, also tried out traditional recommendation of eating croc meat, which we're told by elders that would be a promising cure for asthma.
However, nothing seems to work for her. Please help, we believe surely there should be a capable n kind Dr. or an Md., who could offer us a better n effective remedial suggestions.
IF possible, kindly PM or email us on what we should or should not do, n we would be eternally indebted to you for your kind medical advices or sincere recommendations.:(
Finally, please understand n forgive us, if my wife n me, might dislike or would infact be afraid to receive unsolicitated mails or messages from others, especially when we do not want to be unnecessarily disturbed by undesirable offers. Well intended advice n assistance is gratefully welcome though.

Hi Patrick:) - You're so very kind hearted, we're both appreciative of your effort already. Despite the Feng Shui improvement of changing the bed's position, me wife still find it difficult to sleep whenever the asthma hits her.
Me wife's breathing would be wheezy n she caughs so bad, until the caughing wakes up the kids.:(
Also after the asthmatic attacks, me wife would wake up with a bigger MMigrane, than Cool Hand Luke's one.:D
As I normally sleep like a dead pig:D , it doesn't affect me much, but the kids' got to wake up early for school though.:(

If this "asthma" attack happens only at night and in the bedroom, I suspect something is triggering the attack.
Some people are allergic to dust (and dust mites). another possibility is the air-conditioning. Try cleaning the air-filter screen and see if it works. I have a friend who have this same complaint and he uses an old window unit aircond. My recommendation was for him to change it to a new one. He hasn't, so I don't know if my suspicion is correct.
My point is, if medicines don't help and FungShui isn't any help either, we got to check the surroundings. Go through an elimination process. Firstly, remove the variable of the room itself - ask your wife to sleep in another room (with a different and fresh set of bedding and pillows) for a change and see if she still coughs. Try sleeping without the aircond.

Hi KWChang:) - You're so kind, that probably is the best suggestion that we have got so far, which we're hoping already that this would certainly be better of, rather than visiting another unfamiliar specialist doc, that surely is going to burn bigger holes in our pockets.:)
The airconds is new, now that we think about it, we have not got it washed for some time now.:( Makes me wonder why I am such a lousy in biz of aircond service, forgot to clean our own units.:D
The bedsheet n pillow covers are new though.;)

orchipalar, chang may have a point there. Also, check under the bed and clean/mop it thoroughly because that part of the floor is normally laden with dust.
Also, I have spoken to a friend by the name of Dr Rajen. He is not a medical doctor but he hold a doctorate in holistic medicine and writes for Star very often. I mentioned to him your problem and he suggested you call and talk to his secretary Jasmin at 03 79652715. Tell her Dr Rajen asked you to call. And let's take it from there!

We figure me wife has got too much chemical already in her system. Forgot to mentioned though, her conditions is worsen or hard to be cured, due to the fact that she comes in constant contact with chemicals due to her biz, no matter how much she tries to avoid the contacts, the chemical fumes get into her system.:(

Hair loss can create a lot of anxiety for both young and old. Frequently one's self-confidence or self-esteem depends very much on one's crowning glory. True hair loss is defined as losing more than the daily shed of 100 hairs.
There are many medical causes of hair loss that are remediable, so you don't have to just accept it as "part of old age".
Some of the more common causes of hair loss are androgenetic alopecia, thyroid disease, nutritional deficiency, infections, hormone imbalance & drugs. These causes can be excluded by a thorough history & doing some blood tests, if warranted.
There are many therapy options, beyond that of regaine lotion.
My therapeutic approach depends on the cause of hair loss found.
Importantly, dietary deficiencies have to be addressed with vitamin supplements. Avoidance of physical trauma such as perms, dyes, straighteners, chemical treatments is advisable.
Topical lotions which stimulate hair growth are available. If hormone imbalances are the cause, then oral medications are available.
In-office treatments involves treating with a new laser which stimulates thicker, denser (and blacker!) hair growth. And perhaps application of vitamins after the treatment. This approach has a 90% response rate.

I read from previous posting that it cost 120 per session and 1200 for a series of treatments. Do you charge per session basis or a lump sum for lets say a series of treatments? I enquired from a few beauty centres and they say their treatments come in packages ranging from 2000 to 5000. I somehow feel all these beauty centres just want you to commit to their treatment plan. Could you see visible improvement after 1 to 2 treatments?

I charge per session basis as well as package prices for which I give a
discount. i.e. 120 per session, or 600 for 6 sessions.

You can actually see improvement with every session but of course it is not
dramatic - also depends on how deep you go with each treatment. For red
marks, generally at least 10 sessions are required for good improvement.

Recently, orchipalar raised the problem of asthma. And the kind Dr Lim has taken the initiative to contribute something here, albeit a bit late.
Anyway, chanced upon asthma problem...in your forum. Here's my
unsolicited input:
Asthma is a hypersensitive response to allergens in the environment so one
can only control 2 factors:
1. Dampen the hypersensitive immune response and
2. Eliminate/avoid triggers in th environment. Iin the case of asthma, they
would be aeroallergens.
If one does not want to be "pumped full of drugs", then eliminating
triggers becomes important. But one should not forget that a severe
asthma attack can kill so don't drop the puffers just yet.
Re: controlling your environment - can't control KL environment but one can
control the home environs:
1. Get rid of all carpets, rugs, soft toys (if kids can't part with them,
put soft toy in plastic bag in freezer for 24/24 - will kill the mites),
pets.
2. Vacuum house with one of those new fangled water-based vacuum cleaners I
think OSIM sells them, also other brands, do own research.
3. Mop everyday.
4. Get good quality air filter for bedroom at least (8 hours a day spent
here)
5. There is a thing called Z-net - a bedsheet impregnated with house dust
mite-killing chemicals - it helps.
6. Last resort - shift out of the city.
One should do some research on PROBIOTICS. I believe it helps asthma.
(Try medscape.com) Don't know which brand is reliable though - lots of
overpriced stuff out there.
Last but not least, can do allergy testing but it costs a bomb - don't have
to enrich doctors for this, can visit the laboratory directly. I thnk it
costs upwards of RM600. Cost effective only if one is trying to identify
non-airborne allergens or if one can afford to change one's environment.

Patrick is NOT a doctor.
However, it will be a bit difficult to have a doctor online because we don't have a volunteer and the MMA has a ruling that doctors are not allowed to advertise their services. So a doctor online would be threading a thin line on this ruling.

Please do not misunderstand - it is not our Forum Rules. The MMA is the Malaysian Medical Association.
If you were to read the MMA website article at http://www.mma.org.my/current_topic/advertise.htm you will find doctors are bound by the 'Code of Ethics' of the MMA and the 'Code of Professional Conduct' of the MMC.
However, the above article deals with allowing doctors to advertise their services in line with the encouragement for health tourism. It adds "The MMA has supported the idea of dissemination of basic information. It should be done in good taste without over-publicising a doctor's ability. Basic information such as name, qualifications, specialty, place of practice and even the number of years of experience may be allowed"
So I guess if a doctor volunteers to give online advice, it may be allowed - but without violating our own Forum rules about advertisements.
Thanks for clarifying your earlier posting. I wish to let you know that I did not misunderstand.
I know what MMA stands for and its code of ethics.
My proposal was for a volunteer doctor to provide us answers to medical problems other than" beauty and you" in a different section and under a different heading.
Naturally of course conducting himself within the ethics of the MMA and the Forum.

practitioners finding themselves in any difficulty in deciding upon their course of action
or in doubt as to the safeguards necessary are advised to seek guidance from the
Secretary of the Association.
1. Advertising and Canvassing : The practices by a registered medical practitioner
(i) Of advertising whether directly or indirectly for the purpose of obtaining patients
or promoting his own professional advantage; or, for any such purpose, of
procuring or sanctioning, or acquiescing in, the publication of notices
commending or directing attention to the practitioner's professional skill,
knowledge, services or qualifications, or deprecating those of others; or of being
associated with, or employed by those who procure or sanction such advertising
or publication; and
(ii) Of canvassing or employing any agent or canvasser or distributing visiting cards
for the purpose of obtaining patients; or of sanctioning, or of being associated
with or employed by those who sanction, such employment, are discreditable to
the medical profession and are contrary to the public's interest.
2. Definition of Advertising : The word "advertising" in connection with the medical
profession must be taken in its broadest sense to include all those ways by which a
person is made publicly known, either by himself or by others without objection on
his part, in a manner which can fairly be regarded as for the purpose of obtaining

patients or promoting his own professional advantage, or as appearing to be for
these purposes.
3. Accepted Customs : It is generally accepted by the profession that certain customs
are so universally practiced that it cannot be said that they are for the person's own
advantage, as for instance, a doorplate with the simple announcement of the
doctor's name and qualifications. Even this, however, may be abused by undue
particularity or elaboration.
4. Public Health Medical Officers : Publicity is necessary in carrying out the duties of
medical officers of health and other medical men who hold posts in the public health
or other public services. Provided that this is not used for the individual's
advancement in his profession, this may be rightly allowed.
5. Holding of Public Office : It is the recognized duty of a medical man to take his share
as a citizen in public life and to hold public office should he so desires, but it is
essential that the holding of public office is not used as a means of advertising
himself as a doctor or his professional services.
6. Statement : It is conceded that practitioners may properly place their views on
medical subjects before the public when they can do so with authority. In so doing, it
behoves each one to avoid methods which could be fairly regarded as for the
purpose of obtaining patients or otherwise promoting his own professional
advantage. It should also be remembered that there are many things innocent in
themselves, which may, by the manner or frequency of their doing, gravely
contravene the principles that medical practitioners should not advertise.
Discussions in the lay press on controversial points of medical science and
treatment should be avoided by practitioners. Such matters are more appropriate to
medical journals and for discussion in professional societies.

During my recent trip to a beauty centre, the beauty consultant suggested that i may opted for the vitaminc c treatment which make my acne scar less visible. I do enquiry them about the laser treatment, but according to them , my case I do not need it as it only cater for those suffer the serious matter or deep scarring.

"Re: acne scarring. I don't know what type of scarring Natasha has i.e. deep ice-pick type, saucer-shaped ones or just discolouration on skin due to acne post- inflammatory pigmentation. Her intuition is right, Vit C does not help acne scarring. Vit C is good for repairing sun-damage i.e. wrinkles, sun-induced pigmentation. Vit C cannot destroy fibrous tissue that is causing indented scarring.
Acne scarring can be improved by various other methods - dermabrasion, laser, RF, etc"

To salago and all other ladies, doc respond below may be of interest to you:
Apart from surgery, here are my comments on the various methods available:
1. pills - hormone pills do work but for only as long as you take them. Not advisable because of side-effects.
2. creams - don't work, con job. they only tighten the skin. (Hormone creams are as above)
3. Exercise doesn't increase breast size, exercising the pectoral muscles is good for posture and lifting only.
4. Massage bra - con job.
5. Pumps - Brava does work (infact very wellbut design problems easily broken smartbox, etc. Buy it when they come up with new design.
6. Injections - silicon (done by some beauticians & unlicensed practitioners) - looks good initially but soon after get infection, fibrosis, scarring. Bad idea.
- other stuff they inject - risk of allergic reaction that can kill you. Example recent case of air stewardess who died in the ENT specialist's clinic - she had some unknown substance injected into her breasts - this is what killed her not the nose job.
Are the prices of the listed procedures only for acne scar treatment? Of particular interest to me in the Radio Frequency treatment which I understand can also be used for skin tigthtening of aged complexion. It seems also that at times only 1 session is enough to see results. Would the good Dr Lim still charges for 3 seesions at $1000 or by per session, and how much roughly? Also, how many shots are fired per session? Is the procedure for the whole face or just segments like top half, bottom half or neck? Thanks!

The Radiofrequency you are describing is the Thermage machine called Thermacool - you're looking at paying RM10,000 per session of whole face treatment. Not available here yet but this is what they are charging in S'pore. I have the RF 300 unit which is much cheaper and yes, can be used for skin tightening.
There are no "shots" fired. Price can vary according to surface area.

Vit C injection can be done into the vein (going to whole body) or can be injected onto the face via mesotherapy but usually a cocktail of vitamins is used rather than just Vit C alone - better effect. For IV cost is RM80 per time. For mesotherapy cost varies according to what drugs are used - best combo costs 680 per session.
Facial exercise - there is a video/book out there on the net abt this stuff."

from rm300 per area. Only for upper face dynamic wrinkles (i.e wrinkles
that appear when you're moving face) though. BTA dermalift & mesolift firms

The Doc says "Min sessions - weekly for 4 times, then monthly maintenance. Cost per session 150 each. Recuperation time - nil unless there is some minimal pinpoint bruising which disappears in abt 3 days."

This is not my area of knowledge but for all medical services where an appliance is exposed to body fliuds (blood included), the standard practice is to either destroy the appliance after use or let the patient keep it (if reuseable) for future use by the same patient.
This is because of the danger of reusing the device for another patient. For example, doctors NEVER reuse needles because it can spread diseases like AIDS. Hence it is disposed - so the patient must pay for it.
I would never go to a practitioner who cleans and reuses needles !! Would you?
Thanks for your concern kwchang. Of course I wouldn't want to use re-used needles.
I just learned from acne.org that some practitioners use disposable lancets. This seems to be more affordable for the patients.
Anyway, I think I'll just save up to buy the gadget.

according to her you need to have a post graduate qualification something from the royal colleges in australia or uk and to be certified as a specialist in the malaysian specialist registers.

First, allow me to say that we have never presented our doctor on this Forum as a dermatologist, and I believe neither has she ever claim to be one.
In the doc's own words, she is a "cosmetic physician".
"I have been doing cosmetic related treatment/ procedures since 1998, full time since 2000 (some dermatological , some endocrinological (weight loss) and some including breast enhancement! (Brava)
My qualifications is public knowledge and stand as follows:
1. MBBS (Uni of Melb, Aust)
2. Dip Prac Derm (Uni of Wales, UK)
3. Dip Cosmetic Science (Society of Cosmetic Scientists, UK) -
This is not a "medical degree" - it is the basic degree that
cosmetic chemists (the guys who formulate all the cosmetics
in the market place) have to gain entry into the workforce
of cosmetic R&D labs.
Australia has a society of cosmetic physicians and a society of cosmetic surgeons. France has a course in cosmetic dermatology, just to name one. I can assure you that "cosmetic medicine" doesn't have a "royal" recognition but it certainly doesn't stop a lot of doctors (not just dermatologists and plastic surgeons, gynecologists, opthalmologists, cardiologists, psychiatrists too!) and beauticians from practising "cosmetic medicine" simply because it is lucrative and in high demand.
The lines between cosmetology and medicine are blurred these days and I don't think anyone should have disdain for the highly trained aesthetician who performs low concentration chemical peels or microdermabrasion AS LONG AS THEY ARE PROPERLY TRAINED. They are doing so in US.
Nor should anyone have disdain for any doctor with an entrepreneurial spirit who chooses to practise cosmetic medicine (which involves handling lasers, Botox, etc, etc) AS LONG AS THEY ARE PROPERLY TRAINED.
Where did I get training for such procedures? Just like all other doctors, I attended courses and conferences, both local and overseas. Mostly paid for out of my own pocket and some paid for by drug companies.
I trust the above helps clear the air."
And I would like to add this for the record. When we first spoke to this doctor to assist on this Forum, she kindly agreed as a service to the E-community without hesitation. There was never any request or demand, subtly or otherwise, for publicity or channeling of business to her clinic. This doctor has a thriving business and has more than enough to handle besides being a mother. I would like this doctor on our panel to know how much this website and the community here appreciates the 'service' she has provided so generously. I don't know ALL the brands of life cell/placenta therapy because most of them are not legally registered with the Ministry of Health.
Some ORAL brands are registered with Ministry of Health, but NOT THE INJECTABLE versions. I think there are abt 3 brands of oral ones which are legally registered. You can check it on the National Pharmaceutical Control Bureau website (www.bpfk.gov.my) abt which drugs are legally registered.
There is no hospital or clinic in Malaysia certified to administer life cell injections.
This life cell therapy is famously practised in Clinic La Prairie (no relation to the cosmetic) in Switzerland. It is actually a full-fledged hospital which provides anti-aging therapies which include life cell therapy. The costs are abt RM40,000 for a week' s stay in the hospital where they give you first class health checkups and anti-aging therapies. A lot of international celebrities and rich politicians go there for annual rejuvenation. This is the only hospital I would recommend.
If you can't afford it, you can buy life cell injections directly fron Japan @ a cost of abt RM3,000 for a full course and inject it yourself provided you accept the RISKS. Please don't ask your innocent family doctor or your neighbourhood nurse to inject the stuff for you. You are risking their licence to practise medicine.
Or you could pay the local black market rate of RM15 -20,000 for the course. You can find them in virtually any street corner - doctors, beauticians, accountants, network marketers, etc -all willing to make a fast buck. I have got nothing against these enterprising people but they are peddling something potentially life-threatening.
DANGERS OF LIFE CELL THERAPY:
1. ANAPHYLACTIC SHOCK i.e. severe allergic reaction that usually kills you unless immediate life-saving drugs are administered.
2. LIVER FAILURE - reported inYomiuri Shinbun (Japanese newspaper) recently.
A note abt the oral version - works for abt 6 months to make you feel "energetic", then effects wear off even if you continue taking them. Thsi info is straight from the horse's mouth - a guy who used to own the distributing rights to one of these products. Yes, he had sold off the company when he told me this!
There you have it - the updated inside story on one of the most lucrative trades in town!
P.S. I don't have anything against using a safe drug or supplement even if it isn't registered. Our Drug Control Authorities are too slow to approve even drugs which can save lives. You often hear of people (cancer sufferers esp) spending thousands to fly overseas to get certain drug treatments simply because Malaysian health authorities don't approve of the life-saving drugs. Even Singapore is much faster to approve drugs & supplements.
For those who are young & healthy and wish to remain so, my advice is to stop smoking, eat healthily, exercise regularly and take SOME supplements (do your research on this, not just buy it from the direct-selling company promoting it!) . There is no magic bullet for remaining young ,even if there were, an unhealthy lifestyle would nullify all the benefits of the magic bullet!

recently an article in the malay mail revealed that in singapore GP with special interest in botox laser treatment etc charges substantially less than a dermatologist performing the same procedure.For layman like us the practical implications is that dermatologist obviously offer something 'extra'

Hi Dr. Lim I am 27 years old but people have often said they thought i look older. I am not sure why they say that because I think i look my age.
One thing do the past 2 years i noted my left eyelid has 'shrunken' in. so sometimes when i am tired I have triple eyelid and this bothers me a lot because I can feel the triple eyelid.
I am wondering it is feasible or safe for fillers (hyaluronic acid) to be injected into this upper eyelid?
Just a thought.
By the way I am wondering how much you charge for IPL for skin redness and skin rejuvenation? My skin is naturally sensitive so I have uneven light telangiectasia on my face.

last week a local doctor told me that doctors are not allowed to inject vitamin c to patients anymore since vitamin c is not considered a medicinal practise. Instead it is more like a beauty aid with no proven value/efficacy. She said that she would not accept new patients requiring this injection anymore - it might jeopardise her practising license. However, she lamented that beauty salon operators can continue with injecting their customers with vitamin c - since they are not regulated by the medical authority.
I don't understand this, and it does not seem to be logical - nobody but only doctors can administer injections to anyone
Please comment.

The doc says "Cosmetology per se does not come under the purview of medicine. "Cosmetic Medicine" is not yet a recognized "branch of medicine" in Malaysia."
"Botched jobs" obviously are caused by incompetent people who do things beyond their scope of training and/or ability. If one is foolish enough to go to a beauty salon to have ones nose injected with silicon, then one should always expect the worse of outcome. I didnt want to put this question to the doc because I think the answer is obvious. As to legal recourse, I think that matter be best put to a lawyer - Patrick

The doc also went on to write: If you want to go to a dermatologist for procedures such as laser & botox, then it's your prerogative.
Personally, given a choice of having microdermabrasion done by a trained &
experienced beautician who has been operating the machine for years or
having it done by a dermatologist (there are plenty of dermatologists doing
disease dermatology who have no experience in cosmetic dermatology) who has just bought the machine and never operated the machine before, I would
choose the former.
Would you rather have a hair transplant done by an Australian "GP" who
incidentally conducts training workshops in hair transplant surgery for all
doctors around the world or have it done by one of her students who is a
local plastic surgeon (your royal college type)?
The scope of cosmetic treatment is so wide, ranging from massage type
facials, to chemical peels to microdermabrasion to Endermologie, Botox
injections, lasers, IPL, and more interventional procedure such as double
eyelid surgery, breast implants, etc, etc. All of these procedures has its
place amongst aestheticians, physicians and surgeons. They are all
professionals in their own right. There are plenty of aestheticians and
doctors who do not conduct themselves professionally: E.g. the general
public (every profession included, even insurance salespeople, accountants,
etc etc) who give placenta injections, dubious beauticians who inject Botox
or collagen or worse still silicon. Then there are dermatologists who allow
their nurses to operate lasers and get fined & suspended by Singaporean
Health Authorities. The mistakes of the minority have marred the reputations of genuine operators.
In summary, it's just common sense whom you choose to go to for various
cosmetic treatments. Shop around, ask your friends, seek second, third or
even fourth opinions. You'll find the right doctor or beautician who can do
the best job for your requirements. It's a consumer's market out there.
The health authorities are carrying out credentialling exercises about who can
do certain procedures such as laser & injections. It will be implemented in the near future.

Fact is only doctors & nurses are legally allowed to administer injections.
The article in the newspaper re-emphasized a longstanding rule that no-one (not even doctors) is allowed to use or import drugs which are not registered with the Ministry of Health. There are many brands of INJECTABLE vitamin C which are not registered. But docs can make up their own injectable form from raw materials.
These are the facts and I hope they are clear."

i have a birthmark exactly on my right eyebrow...it is black in colour and it seems it is growing bigger over the years...the size is about 10 cent coin.

i currently still suffering from the problem of pimples and acnes.
i tried many ways but still useless.
now i'm taking tetracycline and the problem is under control, but if i stop taking it, the pimple and acne will come again after few months.
can i continue to take the pills ?
my fren told me a pills called isotretinoin, he said is very effective, so where can i get this and how much?
beside taking pills, any other method to prevent pimples and acnes ?

Yes, tetracycline is an effective antibiotic to control acne. It is not harmful to the body to continue the antibiotics on a long term basis but your acne bacteria can develop resistance to the antibiotic. Hence, i don't usually suggest taking longer than 6 months at a stretch.
Isotretinoin is very effective but there can be serious side-effects - cannot fall pregnant (causes fetal abnormalities), can affect liver and increase cholesterol. Can affect bone growth. Minor side-effects include dryness of skin and mouth, everywhere. But acne also recurs after stopping it for abt 1 year.
Try taking more natural methods e.g probiotics (in yoghurt), vitamins C&E, low carbo diet, plenty water, decrease stress, etc.

I have some ice pick scarrings on my nose and cheeks. I am thinking about punch floats/punch elevation for these scars. Quote:

"Restylane can temporarily fill a distensible (means you can manually stretch it flatter) saucer-shaped scar. No use for ice-pick scars. Duration of action for Restylane is about 8 months to 1 year.

Doc says "Omnilux delivers blue light of 408 nm. It is effective not just for body acne but face as well . Done 2x/week for at least 8 sessions. It does not necessarily work for everyone although I think success rate is definitely more than 50%. Last year I trialled a patient on blue light treatment following their protocol i.e 8 sessions over 1 month - didn't do a thing for her but then again, her cause of acne was endocrinological. If she had to pay for it, it would have cost at least RM2000.
I don't know who in Malaysia has the Omnilux but just be aware that there is a failure rate i.e. doesn't work for eveybody. You'll have to contact the manufacturers about what the failure rate is."

Lately there has been many issues raised about Acne problems and treatment. Our Doctor has been kind enough to pen a a short article specially for the benefit of our readers. Here's her article:
Impact]ACNE SCAR TREATMENTS[/COLOR] - sorting out the plethora of treatments offered out there
One reads of many ads in the papers touting acne scar treatment. What really works?
It all depends on the depth and type of scars - I categorize them into:
1) depressed scars - icepick or saucer-shaped
2) keloidal - more common on anterior chest
3) hemosiderin stains - red discolourations
Below is a list of treatments currently on offer with a brief commentary:
Icepick scars - best treatment would be punch excision but this is realistic only if you have a few of them. If you have multiple densely packed icepick scars, then it would be impractical to do this treatment. Another option is subcision and filler - in theory, it is quite appealing but I've seen some cases where the patient ends up with a lump under the scar which is still apparent. Another negative point is the expense of the filler material (if it is temporary then you 'll have to fork out every year). If it is permanent, there are so many controversies about what these permanent foreign bodies under your skin can do to you. Another filler option is your own fat graft but you'll have to find a plastic surgeon who is expert at doing this because fat grafts do regress somewhat.
More affordable options (but with some downtime) would be needle-abrasion (NO, you do not have to pay RM2000 for the needles) and chemical reconstruction of scars.
Saucer-shaped scars - many treatments abound. Effective ones would be Cooltouch (1320 nm) or Smoothbeam (1450 nm) laser. Plus points -Smoothbeam can also reduce active acne and there is no downtime. Negative points - very expensive (be prepared to spend about RM10K total) and painful (physically, not just on the hip pocket nerve!). Microdermabrasion can help only very, very shallow scars. Needle abrasion (also called collagen induction therapy) works very nicely and it's affordable, negative point - abt 1-2 days downtime.
Keloidal scars - Tried and tested options include intralesional steroid injections (it works but can leave red discoloration) , silicone sheet plasters (totally painless but may not work on everyone). Some newer options include radiofrequency, laser or IPL to "soften up" the scar - does not give consistent results but worth a try if the tried and tested options don't yield results.
Hemosiderin stains - pulsed dye laser gives best results but expensive. Cheaper option - good old chemical peel but must be done in experienced hands otherwise can result in chemical burns.
Just a quick comment on other treatments:
DERMABRASION - it is mechanical sanding of the skin. Not suitable for Asian skin as we tend to develop post- inflammatory pigmentation.
CO2 or Erbium Yag laser - it also removes a pretty thick layer of skin and again like above, can cause post-inflammatory pigmentation
1064 nm YAg laser (often advertised in CN beauty salon ads) - good for fine lines but doesn't work on scars. Don't waste your money.
I've probably confused those who are not familiar with the terms used above but it is very difficult to condense everything into a short email.
The short message is
- prevention is better than cure. One should not wait until it scars before treating acne because a realistic expectation from acne scar treatment is averagely 50% improvement - there can never be 100% reversal of scarred to unscarred skin.
- choose your treatments wisely and budget accordingly (make sure you can afford the full course of treatments) because half-hearted treatments would not give you much results.

"It depends on type of acid used. Generally, acid peeling can be deeper if strong acid is used. Most microdermabrasion machines are built not to cut too deep except the doctors' models."

Pulsed dye laser is very expensive and the good doctor has no plan to invest in one yet.
As to your other question about vitamin C, Doc says:
"Why do you need Vit C injections for? As one has read in the newspapers, it isn't the miracle anti-aging jab that it has been falsely promoted as. Vit C is useful as a supplement for improving one's immune system, metabolism and collagen growth in all parts of body. People who need intravenous injections of Vit C are those who have dietary deficiencies, gut absorption problems and possibly smokers.
However, mesotherapy (mincroinjections of polyvitamins) into areas of hyperpigmentation is effective for treating pigmentation and also for promoting collagen growth."

However, i would still like to know what other options i have to treat them. The largest scar has been around for a 3-4 years now, and is about 1.5cm in length. I have heard of using kenacort injections to treat them. I'm not afraid of needles, but worry about the cost and effectiveness. Is the effect of the injection lasting or would i have to repeat it every few months for the rest of my life? I am still a student, and since it is only a cosmetic problem, and not anything life treathening, i do not want to burden my parents. Thank you for you assistance.

"Hansaplast should be used continuously even if attempting other treatment options which are:
- kenocort injections - very effective, one injection will keep keloid at bay for anything from 2 months to forever - unpredictable. Cost varies from RM60-150 per session depending on number & size of keloids. Keloids may keep recurring. Hansaplast helps to keep it under control.
Other less effective (less consistent result) options - freezing with liquid nitrogen, pulsed dye laser for red keloids.

Microdermabrasion is a procedure that can remove as many layers of skin as the operator wants. I usually limit the procedure to just the top few layers of dead skin. Results are cleaner, more radiant skin and whiteheads and blackheads are also sucked out.
One only needs to go deeper if treating acne scars, in which case, it would be localised.
It's a very safe procedure if done in trained hands. - Doc

I have a couple of questions for Dr.Lim regarding birthmark removal. I have a café-au-lait birthmark on my left calf. It is dark brown, oval in shape and approx 4.5cm in length.
I would like to know how many sessions I would need to undertake before the birthmark becomes ‘invisible’. How frequent are these sessions done? Also, how much improvement can I see after the 1st session? What happens if I do not follow through all the required sessions and remove the birthmark completely? Will there be any consequences such as recurrence?

Elle, Doc says "Cafe au lait spots can be nicely removed by Medlite laser - minimum 2 sessions required, can be more. Can probably see abt 30-50% lightening after 1 session. Some 20% pigmentation may recur therefore repeated sessions are required to totally remove it. Total recurrence is unusual."

We figure me wife has got too much chemical already in her system. Doc says "Mild acne scars/pores - combination needleabrasion (not much pain if anesthetic cream left on 1 hour) and microdermabrasion (no pain at all). Abt 6 sessions each.
Cellulite - mesotherapy .Pain scale 5. Done every fortnightly initially, then monthly."
