I was recently in touch with the general enquiry department of Cellgen LifeSciences (M) Sdn Bhd which supplies Cellabs Sheep or Deer Placenta extract.
I was informed that they are retailing the aforesaid at RM 288 for one month's supply. I'd say they are thus far the cheapest on the market. Am not sure of the efficacy of the products though, am just scouting around for cheaper alternatives, but as good as the expensive ones. I had keloid prone skin, I knew that after a minor accident that had scarred my right elbow few years ago. The keloid had improved after a few years. But to my surprise, maybe after that incident, my skin would develop keloids easily. One of which is the most discomfort and itch will be at my right shoulder, it used to be a pimple. I chose to ignore it as I thought it would go off by itself. But, in constrast, it develop into a harden and darker colour dead skin type which last for a few year then recently developed into a 10 cents coin raised keloid! I had to hide all my sleeveless tops since then :( :(
Please advice. Thanks.

Those who are keloid-prone have to treat injuries early e.g. prevent infection by applying antibiotic creams or taking oral antibiotics if nec, close wounds with good surgical technique (not under tension) & in the case of acne - don't squeeze, minimise trauma. Apply silicone plaster (can buy in pharmacy) early on all injuries after wound has healed.
For established keloids - we can treat with kenocort injections. How many injections required is unpredictable - done every 4-6 weeks. At least keloid will flatten with this treatment. - Doc

Thanks for your reply (all reply via pm). I had visited a skin clinic in KL yesterday. The doc had removed the keloid surgically. He said that steroid injection might not work as the keloid is very big. So, the procedure is done and just cross finger as what the outcome be... sigh... the next appointment is 2 weeks. Btw, Mr Patrick, what do you mean by 'close wounds with good surgical technique (not under tension)?

You should start using silicone plasters after stitches come out. All the best"

Doc's comments: Velasmooth works consistently (i.e. works for everybody) to reduce appearance of cellulite & firm up the skin. The results of circumferential reduction ("inch loss") & improvements in stretch marks is not so consistent - works for some, not for others.
Some people may say it doesn't work if the procedure is not properly done for them. It is very much operator dependent. End point of radiant heat must be reached to get good results as stated before.
I've personally tried it & it feels like a hot massage - quite comfortable really!

To which Doc has replied:
The correct drug for hair loss is Propecia (finasteride 1 mg) , not Proscar (finasteride 5 mg). Not a good idea to take Proscar & divide it into 4 parts - not each part is exactly 1 mg. Propecia is effective for stopping male pattern balding because it attacks the root cause of male pattern balding i.e. DHT. No known Long Term side-effects if taken as instructed. Short -term side-effect problem with erection in 1 % of people who take propecia - side-effect goes away when drug is stopped
The lasercomb is an adjunct treatment that improves follicle health - makes existing hair fuller & thicker but it doesn't make you grow new hair follicles. Hence, it's important to understand that it's a supportive treatment for hair loss, not stand-alone treatment.
Low level laser is very safe - just don't look at the laser beam directly in the eye.

Looks like your wife has chronic eczema. It isn't curable in the sense that it will go away forever.
Eczema is controllable i.e. with anti-inflammatory creams, moisturisers, steroid & non-steroid creams +/- antibiotics & anti-histamines. Each flare will settle with treatment but if exposed to the same trigger, eczema will recur. You can spend a load of money doing allergy blood tests but often you'll be postive to things you can't avoid e.g. house dust mite.
Don't go hunting for a cure - manage, prevent & treat the disease early when it flares. - Doc

Proscar is for enlarged prostates. Propecia is the one for hair loss. Both same drug but different dosage. The studies for hair loss are done using Propecia, not Proscar. "Cure" is not a word I would use for hair loss - "manage" & "prevent" are more correct descriptive terms. - Doc

Doc says : Can try Mederma - but don't think it is as effective as Cicacare - it's
reuseable up to 3 months - so divided by 3 months, it's not that
expensive

Due to the huge $ involve, I want to get second opinion. My friend tried Ellipse IPL from a skin specialist, and it seems very effective as I can see her clear skin after 2 sessions. But it is equally expensive. May I know what is the difference btw these two machines ? Do you have any better alternative suggestion ? Pls help as I have spent a lot on facial & whitening skin care products. Thank you in advance.

E-light +RF claim to give better results for skin firming, but from what I can see, even with the doctors' unit, monthly sessions need to be done to maintain the effect, not just 5 sessions. Don't know how effective the beautician unit is.
Ellipse IPL is a top end IPL machine which gives great results for freckles & sun-induced pigmentation (NOT for melasma tho)
The function of IPL is to remove existing pigmentation but don't forget that additional UV exposure will make pigmention grow again so lightening creams & sunblock are part & parcel of treatment.
Looking good is expensive!
My charges for Quatum IPL are 1000 for 3x (cheaper than Ellipse) - works well for freckles, but not for red discolourations. Ellipse works great for red discolourations too.

I find that my skin is getting more sagging and look really dull each year even after using all the expensive products from beauty counters. Can doc recomend some treatment for me? How much does doc roughly charge for the treatment? I heard IPL works for firming on skin, is that true?
Here is another questions from my hubby. He started to have breakouts one or two years ago on his forehead and cheek area (especially near to nose area). If try to extract it out, is either in crystal form, white or yellow paste (like squeezing out toothpaste). But the forming is still there. It doesnt get red or go away. Currently he is on Reoccutane, I'm just worried on the side effect that can cause to his liver. Plus, he is still having those acne problem after taking it like a month just his face doesnt get oily like he used to. Any alternate choice to cure the acne problem from doc?

1. to preserving youthful looking skin is avoiding excessive sun
exposure & covering up with sunblock +++ & hats, etc
2. use good proven skincare
3. Elite Laserfacial (RM3500 for 5 sessions) for skin tightening if not much
pigmentation / IPL (RM1000 for 3 sessions) if you have freckles. IPL
doesn't give good skin firming but is good for removing sunspots, freckles.
As for your Hubby - Roaccutane takes at least 2-3 months to see effect. And depending on dose. You're supposed to take it from 6-12 months total to get good long-term acne control. Alternatives to acne control really depend on acne severity - oral antibiotics, phototherapy, chemical peels, topicals only if mild case.

Prices of good skincare is more expensive than pharmacy brands but definitely much cheaper than cosmetic brands like Lancome, SK II, Estee Lauder, etc - DOC

It feels warm for up to 1-2 hours. Redness lasts the same amount of time depending on how much pigmentation one has on face. More pigmentation will absorb more energy, hence redder. BUt one generally looks good straightaway after IPL - nice glow to the face - DOC

I've heard of a dermatologist in KL Imbi called Dr Ting. It seems the queue to see him is crazy. A few friends of mine have gone to him, but I still haven't had the time yet. Is that who you went to, Snoopy?
Well, if you compared Dr Ting and Dr Ko from Klang, more ppl going to Dr Ko. But Dr Ting does have his advantage in the sense that he performed all the procedures by himself. Tat's y the queue to see him is extremely long. Clinic like Dr Ko , hires a lot of consultants, some who are not doctor themselves. Prescribing the type of treatment is done by them. And the procedures will be carried out by nurses. Some more complicated one are done by qualified doctor.

Doc: Gentle extraction is fine - too aggressive will lead to scarring &
dyspigmentation.

Latest announcement from our Doctor:
A combination of blue & red light has been shown in many studies to lower acne count by up to 80% 12 weeks after the end of treatment. It also produces faster results compared to oral antibiotics - usually 50% reduction in acne count by 4 weeks as compared to 3 months if on oral antibiotics.
A suggested treatment course consists of
- 8 light treatments
- 2 chemical peels
- 2 microdermabrasions
All treatments are done within 1 month where possible.

Due to numerous requests, Doc has written in a simple guide for those wishing to know more about acne and related questions. Hope you find it helpful...
To those of you who are going from doctor to doctor, salon to salon looking for the acne miracle cure, here are a few pointers to explain why your acne treatment doesn't seem to work for you:
1. Too high or unrealistic expectations
- Key thing to remember about acne is that it is a MANAGEABLE skin problem not CURABLE. ALL ACNE CASES CAN BE ADEQUATELY CONTROLLED. You can remain acne-free as long as you remain under appropriate treatment but once you slack on treatment, acne may flare again.
- Improvement takes time... It takes at least 3 months to see good control of acne - it doesn't happen overnight
2. Using too low dose treatment resulting in ineffective treatment
It's important to follow instructions about how many tablets to take, how much to apply, how frequently, etc
3. Medications having too many side-effects reducing compliance
- antibiotics giving gastric pains & nausea or vomiting
- topical creams which are too irritating & cause too much redness & peeling
4. Too high secretion of sebum on face diluting effect of antibiotic
5. Stopping medications too early or taking antibiotics for too long a duration
If doctor tells you to take it for 3 months, please take it for exactly that duration. Too short duration leads to ineffective treatment. Too long duration leads to development of resistant bacteria.
6. Too many clogged pores & cysts
Physical methods of extraction removal may be required
7. Underlying hormonal imbalance esp for women
Please see your doctor for further testing
9. It may not be acne at all
It could be some other bacteria if conventional acne treatment isn't working for you. Please see the doctor for review
JUST REMEMBER, ALL ACNE CASES CAN BE ADEQUATELY CONTROLLED, JUST HAVE PATIENCE & COMPLIANCE!

DOC: Microdermabrasion is a form of physical "extraction" without causing inflammation & leaving behind marks. Cost of this procedure is RM150 for face & neck.

I have back acne since few years ago,now still hv minor active pimples occur from time to time,causing my back looks ugly with those scars :( quite serious scars. I'm not sure whether microdermabrasion/laser treatment can help. Cox previously i have visited a dr in taipan,he only gave me a cream to apply(i cant remember the name of the cream)it's for controlling oil production dat caused acne. Now my back acne seems reducing,but not the scars :( so sad whenever think about it.

Permanent hair removal is best achieved with a laser (specific for hair removal). It really is permanent but the no. of sessions required varies between individuals.
I use a long-pulsed Ndyag laser called "Cynosure Elite" (google in - lots of hits). Most effective & safest hair removal laser for Asian skin types.

I'm new member to this forum. I hope to get some helps for my skin problem. May i know is Dr.Lim treated for body acne as well? As so far I only seen ppl talked bout face skin problem. I have back acne since few years ago,now still hv minor active pimples occur from time to time,causing my back looks ugly with those scars :( quite serious scars. I'm not sure whether microdermabrasion/laser treatment can help. Cox previously i have visited a dr in taipan,he only gave me a cream to apply(i cant remember the name of the cream)it's for controlling oil production dat caused acne. Now my back acne seems reducing,but not the scars :( so sad whenever think about it.

Post-inflammatory pigmentation after acne can be divided into 2 types - melanin (i.e. brown pigmentation) & haemosiderin (i.e. red pigmentation).
Haemosiderin stains are best treated with either IPL (best is 580 cut off filter) or pulsed dye laser. Haemosiderin stains also eventually peel out with skin renewal. Brown pigmentation can be treated by various methods such as IPL (560 cut off filter), chemical peeling, microdermabrasion, skin bleaching creams, etc.
Pulsed dye laser (585 nm) is good for red marks but doesn't do much for brown pigmentation, active inflammatory acne nor comedones.
Back acne is seldom quiescent, nor consist only of red marks so pulsed dye laser alone takes care of only one aspect of the problem i.e. red marks. Young patients with back acne I see usually still have active acne so treating active acne is first & foremost (prevention is better than cure). However, a lot of patients are still in denial about having active acne & are in a hurry to treat scars & discolouration. My first recommendation therefore is to use a glycolic acid lotion for the back - kills 2 birds with 1 stone - helps remove comedones - and at the same time peels the skin to gently remove discolourations. More severe acne will need oral medication, not just topicals. For those with extra budget, microdermabrasion also helps to remove comedones & peels the skin to help remove discolourations - same function as glycolic lotion above. I don't recommend IPL for the back for various reasons. If one has extra budget - there are a variety of lasers you can use - Smoothbeam, N-lite, V-beam, etc etc the list is endless.
Lasers, IPL, chem peels, microdermabrasion, etc is only for those with extra $$$ & are impatient to see results. Bear in mind that the more interventional the procedure, the higher the chances of side-effects e.g. burns.
I don't think one can do direct comparisons of the monetary value of different modalities of treatments. Microderma & 585 pulsed dye lasers do completely different things. You can compare Candela's V-beam to Cynosure's Cynergy to Laserscope's Gemini because they are lasers of similar wavelengths & treat the same thing i.e. vascular lesions. You can compare Parisian Peel to other microdermabrasion machines available from virtually every continent.

DOC: "Tightening pores isn't as simple as doing regular microdermabrasion. Large pores are mainly due to high activity of oil glands - factors include sex (males have larger pores), genetic predisposition (not something you can do anything about), ageing, chronic sun exposure, comedogenic products & hormonal fluctuations (you may find pore size different depending on the time of the month).
So what you can do to help minimise pore size is:
1) avoid excessive sun exposure, use sunblock everyday, wear hats/use umbrella if outdoors
2) avoid comedogenic skincare products
3) use fruit acids or topical vitamin A creams to encourage removal of dead skin & stimulate collagen to tighten up the skin
4) if you have extra budget you can do microderma or chemical peel or laser or botox to stimulate collagen to tighten skin & reduce oil production.

One reads of many ads in the papers touting acne scar treatment. What really works?
It all depends on the depth and type of scars - I categorize them into:
1) depressed scars - icepick or saucer-shaped
2) keloidal - more common on anterior chest
3) hemosiderin stains - red discolourations
Below is a list of treatments currently on offer with a brief commentary:
Icepick scars - best treatment would be punch excision but this is realistic only if you have a few of them. If you have multiple densely packed icepick scars, then it would be impractical to do this treatment. Another option is subcision and filler - in theory, it is quite appealing but I've seen some cases where the patient ends up with a lump under the scar which is still apparent. Another negative point is the expense of the filler material (if it is temporary then you 'll have to fork out every year). If it is permanent, there are so many controversies about what these permanent foreign bodies under your skin can do to you. Another filler option is your own fat graft but you'll have to find a plastic surgeon who is expert at doing this because fat grafts do regress somewhat.
More affordable options (but with some downtime) would be needle-abrasion (NO, you do not have to pay RM2000 for the needles) and chemical reconstruction of scars.
Saucer-shaped scars - many treatments abound. Effective ones would be Cooltouch (1320 nm) or Smoothbeam (1450 nm) laser. Plus points -Smoothbeam can also reduce active acne and there is no downtime. Negative points - very expensive (be prepared to spend about RM10K total) and painful (physically, not just on the hip pocket nerve!). Microdermabrasion can help only very, very shallow scars. Needle abrasion (also called collagen induction therapy) works very nicely and it's affordable, negative point - abt 1-2 days downtime.
Keloidal scars - Tried and tested options include intralesional steroid injections (it works but can leave red discoloration) , silicone sheet plasters (totally painless but may not work on everyone). Some newer options include radiofrequency, laser or IPL to "soften up" the scar - does not give consistent results but worth a try if the tried and tested options don't yield results.
Hemosiderin stains - pulsed dye laser gives best results but expensive. Cheaper option - good old chemical peel but must be done in experienced hands otherwise can result in chemical burns.
Just a quick comment on other treatments:
DERMABRASION - it is mechanical sanding of the skin. Not suitable for Asian skin as we tend to develop post- inflammatory pigmentation.
CO2 or Erbium Yag laser - it also removes a pretty thick layer of skin and again like above, can cause post-inflammatory pigmentation
1064 nm YAg laser (often advertised in CN beauty salon ads) - good for fine lines but doesn't work on scars. Don't waste your money.
I've probably confused those who are not familiar with the terms used above but it is very difficult to condense everything into a short email.
The short message is
- prevention is better than cure. One should not wait until it scars before treating acne because a realistic expectation from acne scar treatment is averagely 50% improvement - there can never be 100% reversal of scarred to unscarred skin.
- choose your treatments wisely and budget accordingly (make sure you can afford the full course of treatments) because half-hearted treatments would not give you much results.

how efficient do u think are they in treating acne scars and also red pigmentation? based on my research online, it seems to be pretty promising but with an expensive cost....
regarding isotretinoin, actually how long is it supposed to be taken? what about the dosage? it seems that western countries recommend 1-2mg/kg per day whereas dermatologists in malaysia and singapore seem to try to be on the safe side by prescribing low dosage like 10-20 mg/day and take for a long period to reach the accumulated dosage. what's ur view on this?

For those people who have asked & have not dared to ask out of politeness - the answer is "No, Patrick does not get any monetary (or otherwise) kickbacks when readers of this forum visit my clinic. I am really disappointed from our Doctor's feedback about some of the insinuation floating around. Here we are trying to provide a service to this community, literally for free, and there must be some people who start to get suspicious and start to insinuate.
Now, this good doctor of ours who is such a kind and caring person has decided that she will no longer try to diagnose via photos & email descriptions of conditions.
There are people who hope to save some time & money by getting a free opinion. She had in the past tried to help before by giving an opinion based on such & by doing so some people may not be seeing their doctors when they should. There is a Chinese saying "the more one helps, the worse it gets". I may not only be not helping some forum readers, I am also exposing myself to unwarranted criticism", said our Doctor.
Our Doctor went on to say "I am happy to take questions of a general nature e.g. "what can one do abt pores?" & "what is a fractional laser?" but not stuff like "what can I do abt the red dots on my arms?"
"There will always be forumers who may want to play doctor & for example tell people things differently but in the process, manages to make slanderous accusations to mar my reputation."
"But like I said before, I'm trying to emulate your magnanimity & I truly am happy to help but I guess it's time to limit my exposure. I won't let you down but I just have to be more selective from now on because I really can't afford unwarranted criticism."
I fully appreciate the stance taken by our Doctor who had so magnanimously agreed to assist our forumers when I first approached her. And she has been so kind to do all this and more these last 5 years as part of her social responsibility. Yet there are people who just dont appreciate this.
Even worse is now I hear of insinuations of kickbacks to me as the moderator. It can be so demoralizing and humiliating. In all my social and community work, and my lifetime of work career, I have yet to even take a single sen of kickback or whatever. I have always been contributing to society from my purse with whatever I can spare. Including for this site that was founded by Jeff. We have put in so much of our valuable time and money, and never expecting any returns or rewards of any kind. It was just our love and contribution to a "job".
Whilst such insinuations dont really bog me down or spoil my day, I will not hesitate to shut down this Column if there is a need to kill such suspicions or insinuations if it gets too pervasive. I will only feel sorry for those many who really appreciate and have benefitted from our doctor's kind advice.

I agree with Patrick about how kind Dr. Lim has been in taking her time out of her busy practice to answer questions for us forummers. I belong to another acne forum and over there, it's mostly people who have experienced the treatments that answer the questions there. Here in this forum, we only have people asking questions and not giving back to the forum after they have gone through the treatments so we only have Dr. Lim's advice on procedures.
I personally find Dr. Lim a very sensitive and kind person. She talks to you, not down to you, like a lot of other doctors. You can tell she really cares and wants to help improve our appearances. She doesn't push expensive laser machines if she doesn't think it will work for our type of problems. I've gone to her for needleabrasion several times, coming up from Ipoh just for her treatments.
It will be a shame if she no longer answers the questions here as there are many who seeks answers to their problems but are a bit worried about going to a doctor. It took me years of reading the forums before I got the courage to see a doctor about my skin problems. I would love to answer questions sometimes but I am worried that if people take my advice, they may not like the results to the treatment as it's not suited for their skin. So I can understand a lot of people's reluctance to post here. We all have to weigh each advice given by anybody, including the doctor, and decide if we want to go through with it. There's no 100% guarantee on anything and to have a 50% success rate is very good. After years of treatment, I'm still trying to get to that 50%. I think I'm getting close but on some days, I feel really down about my skin ( acne scars ) and think I've wasted money and not have any improvements. Especially for people with acne scars, there are no shortcuts. It takes a lot of treatments to improve the appearances of scars. When we excise a scar, we basically trade a hole for a line. We then have to see how we can minimize the appearance of the line with microdermabrasion or needling.
Anyway, long post but just want to show my support for Dr. Lim and Patrick. As Malaysians, we are quite sceptical if anybody tries to show some community spirit because we think everybody else if out to get something. Treat this forum as a way to get information. Nobody is forcing you to go to Dr. Lim's clinic after you find out what treatments are available. You can shop around and contact other dermatologists and even go for consultations before you decide which doctor you feel most comfortable with. This is a FREE service and it's great!

But dont, like what the Doc said, take what she say as the be all that end all. Consult others. Seek second opinion or even a third.
The Doc is doing very well in her business and certainly dont need to do what she is doing for our forumers. It is her magnanimity that's taking her down our path. Appreciate that please.

I don't have a fractional laser & hence do not have any personal experience using it except for a trial on my own face & seeing results in my clients who have done it.
I shall give my opinion based on a lecture I heard in Sept last year given by the head of laser surgery in Singapore National Skin Centre. I think this is an excellent summary as he has no vested interest in the machines & the no. of cases they treat is large.
He found overall 30-50% improvement in acne scars. He commented that he definitely did not see "90% scar improvement" as quoted by some users. He also added that there were also some cases where there was 0 improvement.
My view is that with correct patient selection, 30-50% improvement is very achievable. It is well-known that rolling scars respond well to fractional lasers, not pitted scars. "Orange peel skin" also do not respond well. Red pigmentation should also disappear with fractional lasers provided treatment is of sufficient depth & density. Each session only "takes out" 20% of skin so to get good results, one needs 20% x5 sessions.
As for cost, Mosaic (Korean) is cheaper than Fraxel (American) because consumable cost (i.e. per shot cost) is lower. But I believe the clinical result is the same if comparable parameters are used. Getting a good result with minimal side-effects are very much operator dependent when you are comparing these 2 lasers. Both are fractional Erbium glass.
Added note: fractional CO2 lasers give better results but a higher incidence of post-inflammatory pigmentation in Asian skin (very problematic).
Isotretinoin dosage
- varies as you have said. Studies have shown that even taking 20 mg twice a week can give good acne control.
- advantage of conservative dosing is lower incidence of bad flare at start of treatment. Sometimes flare is so bad, oral steroids are needed to control it. Have seen bad scarring from high dose isotret - DOC

All these methods of emulsifying fat e.g. radiofrequency (>30 brands in the market), ultrasound, laserlipolysis without suction, mesolipo, etc are methods of targeted fat loss. I.e. it is a method of prioritising fat loss, certainly not a method to replace diet & exercise.
For women, when we go on diets (without exercise), we lose fat & muscle at the same rate - not a desirable result. For some, when we lose fat we tend to lose it first in the face, then the boobs.... resulting in a haggard flat chested appearance but still with big hips & arms. What the machines do is emulsify the fats at the locations treated e.g. get rid of love handles before anywhere else, like the face. The fat goes to the liver to be metabolized i.e. utilised or burnt off. The liver cannot differentiate between fats emulisifed by the machines & fats consumed in a meal. Whatever fat that doesn't get burnt off will get redeposited in the body.
To make these treatments a success, one has to go on a hypocaloric diet or if one desires to eat the same calories, then one has to exercise to burn off the excess fats generated.

As for our other forumers' benefit, doc has replied to suggest " It's really hard to prescribe treatment over email as you can understand. All I can say is that you are on appropriate first line acne treatment for mild to moderate acne. As to whether it is working or not - you really need to give it at least 2 months to START to see some effect. If your acne is more severe than moderate, then you need to be on more aggressive intervention -something I cannot conclude without seeing you face to face. If the above is correct, my question is :
1) What differentiate these scarred tissue from normal tissue?
2) Does treatment of acne scar required destroying the fibrous scar tissue in order for collagen stimulation? Is that what fractional laser do? Selectively destroy certain layer of the skin?
3) Since Retin A stimulates collagen growth (as seen in wrinkle treatment ads), does it help in stimulating collagen in acne scar (eg ice pick, boxcar,). Would doctor recommend using Retin A for acne scar?

Here's Doc's respond to your questions:
1) If you look at a cross-section of scarred skin v. normal skin under a microscope, the collagen in normal skin is arranged nicely, something like bricks laid nicely in rows. Scarred skin is in disarray.
2) Fractional ablative lasers (FCO2) "cuts" the scar tissue & the accompanying heat stimulates new collagen to rebuild smoother skin.
3) Retin A is useful after fractional laser to rebuild collagen, yes. But, by itself, without fractional ablative lasers, is not useful for depressed scars - see no.2)

Firstly, placenta jab is not going to remove pigmentation.
Secondly, Melsmon brand is not registered in M'sia (that's why only beauticians peddle this brand) so you don't even know if you are getting the bona fide stuff.
3rdly, there is a risk of an allergic reaction. If you're unlucky enough to suffer a severe reaction, you could very well die in the salon. Remember the air stewardess who died in the ENT clinic in town about 3 years ago?
Just stick to safe methods of pigment removal.

- almost half a year ago, a doctor did laser on that keloid and told me it will flatten out after the scab has fallen off, but after that, it seems that that's no effect at all. It just stay the same
- is steroid injection the best treatment for this?
- what's the possible effect after an injection? that area will get swollen? reddish? what's the downtime one would expect after an injection?
- will this worsen the scar and possibly makes it grow bigger? Is there a risk on having the injection or is it best to leave it as it is?
- how much does it costs per injection?
2. when i pull my lips down, it seems that there are around 3 small red veins under my nose just below the nostril (bottom corner of the nose). There are some tiny ones which is hardly noticeable. Is this called broken capillaries or is it supposed to be like that at that area?

- Any downtime associated to this? redness, swollen skin?

1. My face is oily on T-zone area and very dry and sensitive on the rest of the skin. I understand that not all off the shelf products are good for your skin. So which ingredients should I look at to avoid when buying facial products e.g. cleanser, toner, moisturizer.
I also saw on Oprah that we should avoid ingredients like paraben, fragrance (this is almost impossible to avoid), phthalates, germall, aluminium (also impossible to avoid for deodorant. As for the "trapped hairs" - sounds like ingrown hairs. Constant shaving can aggravate this problem. Laser permanent hair removal is the only way to cure this problem.

Your questions are a bit vague. You didnt mention number of spots, location etc and shape and contour of what?? Anyway, managed to get an indication from Doc that microdermabrasion for face is about RM120. I strongly suggest you make an appointment to see the Doc, get advice and option, quotations, then decide from there. Does that help? - Rgds, Patrick

To your question, Doc's response:
Pimple scars can be divided into different types
1) Merely post-inflammatory discolouration (red & brown) -we can use microdermabrasion, peeling ,skin lightening creams
2) keloidal - if early can try scar gel, if chronic, will need steroid injections
3) atrophic scarring (depressed) - needleabrasion would be good or laser (for selected cases)
To determine your type of scars, I strongly suggest you make appointment for Doc to take a look at them. Then you can take it from there as to your best option. All the Best.

It is very unlikely our Doc will estimate the charges for the services you enquire. She just doesnt believe in making quotation that may not be fair or applicable and would certainly prefer to be able to look at you, give you appropriate advice before making and estimated quote. I have all types of pigmentation & freckles ( due to ageing and sun exposure) all over my face, esp. the cheeks. I have to use concealer to cover these darkspots on top of face makeup powder/foundation . Can this be removed under your Medlite laser ? How much & how many times ? I know is difficult for Dr. to quote without seeing me. I just do not want to waste Dr.'s precious appointment time if the package is far beyond my budget. Any standard range of package ? :confused:

If it's just freckles - a single session of Medlite laser will suffice to
remove all freckles - cost can vary from 100-500 (face full of freckles). Solar lentigines may need
1-2 sessions.
However, if face is full of dots everywhere, the most economical way of
removing spots is IPL - RM400 full face.
Regardless of which treatment is selected, please note that aftercare (i.e.
prevention of pigmentation recurrence) is the bulk of the expense. If you
are not diligent about applying sunblock & skin lightening creams,
pigmentation recurs with sun exposure (the cause is UV so it makes sense to
avoid cause). So many times, I get asked about "guaranteed removal packages"
- prevention is the key - no such offer as "guaranteed removal packages".

My mum has age spots which are getting darker and wants them removed. Rather than risking it with any available beauty treatments at any beauty shops, it is much safer to have them removed under the hands of a medical doctor.
In order not to waste Dr Lim's precious time, I have emailed her a picture of my mum's condition with areas of concern highlighted for her attention. She has promptly replied with the appropriate procedure with reasonable cost.
She is indeed grateful that I emailed her the details first prior to making an appointment as no time was wasted for both of us (also petrol). Perhaps those interested in her professional services can render the same courtesy and consideration as she is one busy doc.

I believe its best you meet up with our Doc or like someone here has done is to send Doc a pic of your situation. It's really difficult to estimate cost because it depends very much on each particular situation and extent of treatment required. Perhaps you could also read the Doc's blog for some idea and then write or call her. I have pm you the relevant details. Regards.

That's very smart of you!! I hope the other Forumers will follow your initiative for mutual benefit. Yes indeed our Doc is very, very friendly but also extremely buzzzy.
Btw, your mum's dark spot shouldnt be a problem to deal. A few of our family friends were treated by our Doc too and they were very happy with the results. But be advised. One has to look after one's complexion afterwards to prevent recurrence.

Yes my friend. Cannot advertise per se is true. But really, advertising is least on the mind of our Doc when we started this column. This Doc often has more patients then she can handle, and having a good, loyal assistant is a bit of a problem.
Anyway, we started this column with the objective of providing a "service" to our forumers for the sole purpose as a kind of "corporate social responsibility". Nonetheless, it does have a beneficial spin off effect for the Doc and that we cannot deny. But the original and main obective of this column was as a benefit to our forumers. Full stop.

Minor acne breakouts don't warrant stronger medications. Besides, Bactrim is a perfectly good antibiotic that works great for acne if one is not allergic to it.
Steroids are never used to treat acne except in the rare situation of acne fulminans (superbad eruptive acne).
Forget abt Vit C jabs - they don't help acne.
You may want to add Retin A cream but if your skin is very sensitive skin, be careful with it.
Can't rush acne, whatever you take or do, it will take at least 2 months to settle. If you want to improve the appearance a bit quicker, do a chemical peel - Doc

The price is expensive,I have done fractional co2 the same laser as Dr Lim at Dr Ranjit skin specialist for the whole face including cream and medicine for only RM500.00 each time.And he is a dermatologist.

I'm 22 and I have several ice pick and box acne scars on my both cheeks and a few on forehead. I just did 10 sessions of diamond microdermabrasion last year with Dr.Chea but it did not improved the scars much. Which was very dissapointing.
I've read about punch excision to treat the scars, and I wonder if Dr.Lim have any experience in that area. If she do, mind to elaborate on how the process will be done and how much will it cost?

Doc says Microdermabrasion is not effective for acne depressed scars - only useful for lightening discolouration & cleaning out pores.
Ice picks are best treated with TCA CROS or if really deep punch excision. Punch excision is only realistic if you have a few (<5) ice picks - you can't be cutting out your entire face. Punch excision involves cutting out the scar like a cookie cutter & then stitching back the hole. So you end up getting a flat line scar instead of a hole. In any 1 session, we can only do 5 max. Cost RM350.
After that we can do laser resurfacing to smooth out the line scar 2-3 mths later (separate cost though).

Doc's reply:
Mosaic (brand name) is a fractional Erbium Yag laser i.e. fractional non-ablative laser. In other words, it's less invasive - downtime is only 3 days of redness. The trade-off is it's less effective in terms of treating acne scars.
Fractional CO2 laser is a fractional ablative laser i.e. fractional ablative laser. i.e. more invasive. Downtime is 1 week of looking "sunburnt" before skin peels off. Trade- off - more effective as it goes deeper.
I do only fractional CO2 laser. Charges depends on depth of treatment form RM300-800 per session.
